Fill out the form (indicating MELROSE YOUTH SOCCER as your Club Name) and submit to
complete your CORI obligation.
Instructions for completing CORI form
An important note to all Adults affiliated with Melrose Youth Soccer:
As you may be aware, the Massachusetts state legislature recently
changed the rules for all organizations that serve children to ensure
the protection of Massachusetts children. The rules madate that
nationwide CORI checks be performed on all adults affiliated with
organizations that serve children.
Here is what you have to do in order to help Melrose Youth Soccer comply with the new laws:

- You must only fill out this CORI form once in your career as an
MYS soccer coach, but
- You must register online as a coach every season, so that
you officially indicate your intent to coach. We make rosters from
the online data, and you will only be placed with a team if you
register online.
